That is less because clinics price the work arbitrarily and more because several operations of quite different difficulty sit inside the one name.

An epicanthoplasty revision is not a single fixed procedure. The scale of the operation changes with how far the scarring and adhesion from the earlier surgery have to be released, whether the fold at the inner corner has to be rebuilt, and where skin has to be brought from when there is not enough to cover. Under the same name sit cases settled in an hour and cases that call for reconstruction layer by layer.

What follows looks at what actually sets the scale of an epicanthoplasty revision, and at the grounds worth asking about in consultation.

Why is a revision harder than the first operation?

A first epicanthoplasty works on tissue that has not been touched. Skin, muscle and fibrous tissue are still divided into their original layers, so the border between what to open and what to leave is clear.

A revision reopens a site that has already been incised and sutured. Through healing, layer has fused to layer and the borders have blurred; scar tissue is firmer than normal tissue, so the same extent takes more time and more handling.

Individual variation sits on top of that. According to a plastic surgery report analysing the shape and size of the fibrous band at the medial canthus, differences in that band govern the degree and the type of epicanthal fold. Even after the same operation, a different starting structure means a different starting point for the revision.

How do adhesion and scarring change the extent of surgery?

An adhesion is a state in which layers that should move separately have stuck together. The wider the area involved, the more surface has to be dissected, and throughout the dissection a judgement has to be made continuously about where scar ends and tissue worth preserving begins.

This is not an incidental step. Approaches that use a pretarsal fasciomuscular flap or a graft to deal with adhesion in revision surgery are reported separately in the plastic surgery literature — handling adhesion is a technique in its own right.

That does not make wider dissection better. A craniofacial surgery report covering 421 epicanthoplasties notes that dissecting more widely than necessary can leave a hypertrophic scar or a web-shaped deformity. Where to stop, between opening wide and opening sparingly, is itself the difficulty. Where a depressed scar has already settled over a wide area, restoring it in stages can be the safer course rather than covering it in one go.

How does a second revision differ from a third or beyond?

Two things diminish as the number of operations rises: the skin available to work with, and the amount of tissue still in normal structure. Scarring, by contrast, accumulates layer by layer.

At a second revision it is often still possible to return the remaining tissue to where it belongs. From a third onward there may not be enough tissue left to return, and the plan shifts toward reconstruction — bringing in from elsewhere what is missing. The name is the same; what happens in theatre is not. Why a revision falls short usually begins with misjudging how much of that tissue is left.

What is added when there is not enough skin at the inner corner?

Where a great deal of skin was removed at the earlier operation, releasing the scar leaves nothing to cover with. Suturing alone cannot finish the work, and a step is added: moving neighbouring skin across as a flap, or rebuilding the border between mucosa and skin.

Reconstruction of that kind is treated as a separate procedure. A method for unfolding and rebuilding the fold and a method for restoring the epicanthal fold itself are each reported in the plastic surgery literature. A straightforward revision and a reconstruction differ in what has to be prepared and in the time they take in theatre.

What decides the type of anaesthesia and the length of surgery?

Where the dissection is narrow and no reconstruction is added, the work is sometimes carried out under local anaesthesia. Where the dissection is wide, or several layers have to be handled separately, holding a tense position for a long time is difficult and sedation comes into consideration.

A change of anaesthesia does not change the operation alone. It brings with it the staff who administer it, the monitoring equipment, and the space and time needed to recover afterwards. That is one of the reasons an operation of the same name is described differently from clinic to clinic.

Are there cases where the crease has to be corrected at the same time?

When the shape of the inner corner changes, the point where the crease begins moves with it. So where the line is pressed down by the corner, or where it is expected to sit out of true after the revision, the two are designed within one plan.

Where the outer side of the eye has given way as well, the scope widens further. If the structure at the outer corner has collapsed and mucosa is showing, restoring only the inner side leaves the two sides out of balance. What is done at once and what is divided is decided by how much tissue remains.

What to establish in consultation

Before comparing what different clinics say, the thing to settle first is which of the two your own eye belongs to. If one account describes releasing adhesion and another includes reconstruction, the two are not describing the same operation to begin with, whatever figure is attached.

So two questions help in consultation: whether your case is a straightforward revision or one that needs reconstruction, and what that judgement rests on. A clinic that can explain the grounds can also explain the scope.

How far the adhesion extends at present, and what that assessment is based on

Whether your case is a straightforward revision, or needs skin moved across

Which operation this is in the sequence, and what is possible with the tissue that remains

Whether it is local anaesthesia or sedation, and why that was decided

Whether the plan is to finish in one operation, or to divide it into stages

Setting the accounts side by side, you will usually find that the point on which they diverge is one of those five.

We understand how wearing it is when something you expected to end with one operation has stretched on, and you no longer know where to start asking.

She One Plastic Surgery practises eye surgery only, and handles revision of inner and outer corner surgery together. We do not recommend a scope beyond what is needed. Through a one-to-one assessment we analyse the remaining tissue and the state of the adhesion objectively, and propose only a plan this eye can take without strain — so that you can decide after understanding what was judged, and why.

Individual recovery times and results vary, and side effects can occur.
