How the proportion between the lower and the outer opening is set is what governs that. The two pull in different directions and work on different structures, so opening too far in one direction alone can throw the eye’s width-to-height proportion out of true.

Many patients combine the two in the hope of a large, open eye. But widening the scope without regard to the skeletal limits of that particular eye can disturb the harmony of the result or bring functional discomfort.

A lower-corner opening brings the lower lid down and adjusts the vertical dimension; an outer-corner opening moves the lateral canthus outward, horizontally, and lengthens the eye. The two act along clearly different vectors, so carrying out either alone without looking at the present proportion leaves room for the balance of width and height to be lost.

What follows covers the anatomical limits to consider in a combined opening, and why the proportion matters.

Which eyes need a lower-corner opening?

A lower-corner opening brings the outer part of the lower lid down and adjusts the angle of the corner. Unlike an outer-corner opening, which widens the eye from side to side, it works on the vertical and on the angle, so the impression each one changes is different.

It can be considered where the following apply.

The outer corner rises so that the impression reads cold.

The eye looks round and short, so an outer-corner opening alone would only add length.

An outer-corner opening has already been done but the eye still reads as severe.

Where the following are closer to the case, the width of a lower-corner opening is limited, or other approaches are looked at alongside.

The outer corner already sits low — bringing it down further makes the eye heavy.

The lower lid has little holding strength — what is brought down can return or separate.

The globe is on the prominent side — more white can show.

A lower-corner opening is not decided on its own. How far it comes down is settled together with how far the outer corner is opened, and when those two are out of proportion the eye grows larger but the impression reads as odd. What follows sets out what that proportion is decided by.

What a lower-corner opening actually looks like

Cases from our official Naver blog. Extending the tissue by force where that space is short can expose conjunctiva or leave a scar that shows. Measuring the distance between bone and globe in millimetres before surgery, and setting an appropriate range from that, is therefore required.

Balancing the horizontal and the vertical. East Asian faces often have an eye that is short in both dimensions. Widening only the vertical with a lower-corner opening, where the horizontal length is not sufficient, can leave the eye looking round from top to bottom and the overall proportion somewhat odd. Conversely, an outer-corner opening alone on an eye of narrow height can sharpen the look, so balancing the two directions matters.

Questions we are often asked

My outer corner sits high. Can a lower-corner opening alone bring it down?

Where the bone at the outer corner itself sits high, pulling only the lower lid downward can bend the line beneath the eye into a U shape. In that case the position of the canthal tendon has to be moved gently outward and down alongside, so that the whole curve below the eye runs on naturally.

Can the proportion change again as time passes?

Soft tissue generates tension as it heals, trying to return to where it was. The incised area contracts slightly as a result, and the early proportion can shift somewhat. Allowing for that at the design stage, the amount of contraction is anticipated and the canthal tendon is fixed to the periosteum, so that the intended proportion holds after recovery.

How is the amount decided for each opening?

It is decided by assessing the size of the orbital bone, how far the globe projects and the shape of the epicanthal fold together. Opening beyond the skeletal limit can bring dry eye or lifting of the mucosa. So the range that can be opened safely in structural terms is established through objective examination, and the design is set within it.

Designing with the balance of the whole face in view

A natural corner opening is not confined to widening the area of the eye; it begins with a design that takes in the proportions of the whole face. A small difference in the angle of the outer corner affects the whole impression, so looking objectively at the following before surgery is advisable.

Measuring the physical room between the orbital bone and the lateral canthus

Assessing how far the globe projects, or how deeply it sits

Setting the present width and height of the eye against the target proportion

Analysing the slope of the canthal tendon

Establishing the history of earlier eye surgery and the state of any scar adhesion

Eye surgery that settles the mind

She One Plastic Surgery practises with a focus on eye surgery and eye revision surgery.
