The complaint is the same — the eye looks small — but where the cause differs, so does the part that has to be addressed first.

An eye can look small because the gap between the eyes is wide, because the eye is short across or down, or because the power that lifts the lid is weak in itself. Deciding on a lower-corner opening from the impression of smallness alone can leave the heaviness that came from weak lifting exactly as it was, so the eye does not open up as much as expected. Which problem comes first has to be established.

Two structural reasons an eye looks small

How far the lid covers the iris depends on the levator muscle and Müller’s muscle, the accessory muscle that lifts the lid a little further. Where that power is weak, the iris is not fully uncovered even on an open eye, and the face reads as sleepy or flat through the day.

Where the muscle works normally but the eye is short vertically, or the outer corner tilts upward, the area of white on show is narrow regardless of lifting power, and the eye looks small for that reason. A lower-corner opening works on the conjunctiva inside the lower lid to strengthen the downward pull of the muscle beneath the eye. It addresses the length problem.

So an eye that looks small is not one problem with one cause. It divides in two, and the approach divides with it. Both can be present at once, and correcting one alone leaves the other where it was.

Before-and-after photographs in this post are published with the patient’s own consent, as required by the Medical Service Act.

What a lower-corner opening actually changes

A case on our official Naver blog, shown in photographs.

The operation that changes the lower lid line — see it drawn out ›

Telling lifting power from length at consultation

The measure for lifting power is how much of the iris is uncovered. On a relaxed open eye, around 80 to 90 per cent showing is read as the usual range. Where that is lower, or where there is a habit of raising the brow to get the eye open, ptosis correction is the thing to consider first.

Length is read from horizontal width against the distance between the eyes, and from the angle of the outer corner. Where the iris is uncovered normally but the corner tilts up, or the eye is short across and down so it reads closed-in, the cause is structural length rather than muscle function, and a lower-corner opening is usually the approach.

Where both are present, the two are planned together, but in a set order: lifting power is brought into range first, then the length is worked on. Extending the length while leaving the lifting power as it is widens the white on show but leaves the heavy look, and the two sit oddly together.

What changes below the eye

What a lower-corner opening changes is where the lid margin sits. When the lower lid towards the outer corner comes down a little, the white beneath it widens, and a line that ran straight comes closer to a gentle curve. The horizontal length is unchanged and the opening is vertical, so the same eye reads as less severe.

The worry that comes up most often is the roll of muscle under the lower lashes. A lower-corner opening is not an operation that removes it. When the lid margin comes down, though, the fullness below can stand out less by comparison, so it can feel as if it has been reduced. People who have a pronounced roll and people who do not take different impressions from the same change, so how far down to go is agreed at the design stage.

One more thing worth knowing is that the result is not fixed. The point that was secured tends to draw back a little over time, and how far it draws back differs between people and is hard to predict. Where the two sides do not return at the same rate it can read as uneven, so the design is set against how it will look some months later rather than how it looks immediately after.

Gauging for yourself whether it suits your eye

A lower-corner opening tends to help where the line below the outer corner runs straight down so the face reads sharp, or where the eye looks long across and closed-in with it. If the iris is uncovered well on a relaxed open eye in the mirror and the eye still does not look open, the lower lid line is what gets looked at first.

There are also cases where something else has to be checked first. Where the iris is covered, or the brow has to be raised to open the eye, lifting power comes first. Where a good deal of white already shows below, bringing it further down may not help. Where the support beneath the lower lid is weak, the position is hard to hold, and it is looked at carefully.

These are things to gauge, not a diagnosis. Which of the two weighs more cannot be settled from a photograph, and results differ between people.

Before-and-after photographs in this post are published with the patient’s own consent, as required by the Medical Service Act.

Evening up with a first lower-corner opening

A case on our official Naver blog, photographed through the course.

A first lower-corner opening alongside an inner-corner restoration — 17 photographs at three months ›

Questions that come up

Q1. Can the two be done at the same time?

They work on different areas and different muscles, so they can be done together. How much the lifting is adjusted and how far the length is extended have to be matched to each other, though. Over-adjusting either throws the balance of the eye out, so a diagnosis that looks at both together comes first.

Q2. Why does an eye look closed-in when the iris shows well?

Lifting power can be normal while the outer corner tilts up or the eye is short across and down, and the eye still does not read as open. That is structural length rather than muscle function, so the approach is a lower-corner or other corner opening rather than ptosis correction.

Q3. Is it true a lower-corner opening alone can make the eye look smaller?

Where lifting power is weak and only the lower corner is opened, the white below increases while the top stays covered by the lid, and the proportions can read as off. In that case bringing lifting power into range first is what is advised.

Q4. What should be checked at consultation?

How much of the iris shows, whether the brow is being used to open the eye, horizontal width against the gap between the eyes, and the angle of the outer corner. These four are read together rather than separately; that is what separates lifting power from length.

Before-and-after photographs in this post are published with the patient’s own consent, as required by the Medical Service Act.

A lower-corner opening draws back over time

A case on our official Naver blog, photographed through the course.

Evening up a lower corner that had drawn back on one side — 55 photographs at two months ›

How the two differ during recovery

Ptosis correction adjusts muscle function itself, so early on there can be swelling along with a sense that the eyes open unevenly. That usually settles over some weeks. A lower-corner opening is made through the conjunctiva, so visible swelling is relatively slight, but the sensation of the lower lid being drawn also takes time to get used to.

Where the two are done together, each recovers at its own pace, so the proportions can look off early on and come into balance later. Rather than judging the result from how it looks immediately afterwards, what matters is working out at the design stage how the weight is split between lifting power and length.

Coming away from a consultation having been recommended a different operation than you expected, for the same complaint, is a confusing place to be. Nothing is recommended here beyond what is needed. The proportion of iris on show, the angle of the outer corner and the horizontal width are read directly to establish whether it is lifting power or length, and only the order the eye can take is proposed.

The before and after photos featured in this post are shared with the patient’s explicit consent in accordance with the Medical Service Act.
