Carry the same list of questions from clinic to clinic and the explanations start to differ, and it becomes hard to know whose account to trust.

That is because the three become difficult at different points. An inner-corner restoration turns on how much skin and ligament is left. Rebuilding an outer corner turns on where the lateral canthus can be anchored. A crease revision turns on how far adhesion and scarring reach. “A clinic with a lot of experience” does not separate those three, so it helps to split the questions by procedure.

Why the point of difficulty moves

Difficulty in an eye revision comes down to how much of the tissue used in the first operation is still there, where that tissue can be fixed, and how the adhesion and scarring left behind are handled. At the inner corner the skin and the medial canthal ligament have already been repositioned, so the amount and direction of what remains has to be read before any plan can be set.

At the outer corner the point that held the canthus has often slackened or shifted, so where the structure is anchored again becomes the deciding variable. In a crease revision, the extent of adhesion and scarring around the previous incision sets how much room there is to place a line at all.

What to establish at consultation, by procedure

Inner-corner restoration

Ask first how the remaining skin and ligament are assessed, and by what method the exposure of the lacrimal lake is corrected. Where the inner corner has hooked or scarred, a specific account of how the tissue inside the incision will be repositioned is what lets you gauge the direction of the restoration.

How the remaining skin and ligament are assessed

The specific method for correcting exposure of the lacrimal lake

Whether the existing scar goes inside the new incision

Rebuilding an outer corner

Here the approach divides by whether the opening fell short or went too far and left conjunctiva showing. Hearing where the canthus will be anchored again, and where the tissue to cover exposed mucosa is drawn from, is what lets you judge how far a rebuild can go.

Whether it is under-opened or over-opened

The point and method of re-anchoring the canthus

Where the tissue for covering exposed mucosa comes from

Crease revision

A crease revision changes with its purpose — lowering or moving the line, excising a scar, or correcting the lift at the same time. An account of how the adhesion around the previous incision is released, and how the relationship with the levator is re-established, is what reduces the chance of the line dropping again or settling unevenly.

Whether the existing incision is reused or a new line is designed

How far the adhered tissue is separated

Whether ptosis correction needs to be done at the same time

Questions that come up

Q1. Can all three be consulted on at one clinic?

Yes. Keep in mind that the items to establish differ by procedure, though, and check whether the remaining tissue, the anchor point and the extent of adhesion are each explained separately rather than together.

Q2. Is “it can be corrected” enough of an answer?

No. More than the conclusion that correction is possible, what matters is being told which tissue is handled and how, to arrive at that conclusion. Where a result is asserted without an account of the route to it, it is worth asking again for the specific basis.

Q3. If the scar is barely visible, is the revision easier?

Not necessarily. A scar can look slight while the adhesion beneath it and the position of the anchor point are nothing like what shows. Establishing the state of the structure underneath counts for more than the size of the scar you can see.

Q4. Does the timing differ between the three?

The time tissue takes to stabilise after a first operation differs by procedure, so the point at which a revision can be considered differs with it. Waiting out the swelling and inflammation and letting the tissue settle is safer than moving early.

Q5. If several have to be done together, what else should be established?

Where an inner-corner restoration, an outer-corner rebuild and a crease revision have to be combined, ask for the order and for how each one bears on the result of the others. Moving tissue in one operation changes the tension and the shape at the other sites.

How the course runs afterwards

Recovery also runs differently by procedure. An inner-corner restoration needs time for the skin around the incision to find its place; an outer-corner rebuild for the re-anchored point to stabilise; a crease revision for the released tissue to adapt to its new line. It helps to be told the first few weeks of swelling and the months of settling as two separate things.

Going from clinic to clinic with the same list of questions and coming away without an answer that fits your own eye is a wearing place to be. The three are not explained here as one thing. The remaining tissue, the anchor point, and the adhesion and scarring are read separately for each procedure, and only the direction the eye can take is put forward — so that the differences are yours to weigh before you decide.

Individual recovery times and results vary, and side effects can occur.
